Care
Coordination
Care coordination services help
frail older and disabled adults live safely and independently
at home. Huntington Senior Care Network care coordinators are
Bachelor's/Master's degree and L.C.S.W. professionals who
assist with problem solving and linkage to community-based services
such as meals, housekeeping, day care and transportation. They
have extensive experience working with senior and disabled populations
and their families.
A variety of public and private
programs are available to assist those of all income levels and
degrees of frailty. These include the Multipurpose Senior Services
Program (MSSP), CareLink, which is the Los Angeles County Area
Agency on Aging Integrated Care Management program, and
our fee-for-service program Community Options.
Services are available on a short or long-term basis.
Care coordinators can:
- Conduct
a professional in-home evaluation of needs.
- Assist
with solving care problems and reducing healthcare risks.
- Arrange
in-home help, meals and other services.
- Coordinate service delivery
and monitor progress.
- Provide counseling in support
of life transitions.
- Review financial, legal and
medical issues and recommend actions to take.
- Educate about services and benefits
such as Medicare and Medicaid.
- Act as a
liaison for families at a distance and alert them to problems.
- Assist in
moving an older person to or from a retirement or nursing home.
