that has helped older and disabled adults and their families remain
healthy and independent since 1984.

Center specialists provide free telephone information
and assistance on aging needs and community resources that
can allow older adults to stay in their own homes and neighborhoods.
- Bachelor's/Master's
degree care coordinators
offer families and older adults short and long-term assistance
to identify needs, develop a plan of action, obtain necessary
services and monitor care.
- Caregiver
support services help local and long-distance family
caregivers manage stress and ease the burdens of caregiving.
- 50+
Health Connection, a free membership program, sponsors
education programs that promote health and wellness for
older adults and family caregivers.
- Geriatric
Assessment Clinic services can help determine an
older person's medical, psychosocial and functional capabilities
and recommend treatment.
- Perspective
